How to Register
If you would like to register at the White House Surgery, you can do this online or by collecting registration forms from our reception team.
We will ask you to complete a GMS1 form and a new patient questionnaire.
Find your NHS number here, contact the surgery on 0114 2245878 or attend the surgery and ask a member of our reception team.

A Patient Guide to GP Services
Communication Needs
If you are a patient who has communication needs such as dyslexia, needing easy read documents or a BSL/language interpreter please record this within the section of your new patient registration form.
If you are an existing patient with new communication needs; please inform our reception team who will ensure this is recorded in your patient files.
Right to request a GP
On registration you with automatically be assigned one of our partner GPs as your usual clinician. This does not mean you need to see this GP, but can be used for generic health forms. Whilst at the practice you have a right to choose your preferred GP. The practice will try to honour this request but advise this may not always be possible due to urgency and scheduling. If you have a preferred clinician, please make the reception aware at the point of booking so we can try to accommodate this for you.
