If you require information about test results, please telephone after 10:30am and speak to the reception team. Some tests may take up to 7 days to come back, others more.
To maintain confidentiality, test results can only be given to the patient concerned, unless specific consent has been given to share information with a nominated person.
What can the receptionist tell me?
The receptionists are only able to give limited information about test results, depending on what the clinician will have noted when they were received. If the clinician has commented that they are normal, the receptionist can tell you this.
What if there’s a problem?
If there is a test result that requires urgent action, the practice will try to get in touch with you. Initially this would be by telephone, otherwise by letter. It is important for you to notify us of any change of your contact details.
Why do I need to repeat the test?
The doctor may request that you need to repeat a test, if they require further information before seeing you to discuss the results.
The hospital has sent me for tests, can you inform me of these results?
If a hospital clinician has requested tests for you we advise that you contact them directly for the results as they will be the most appropriate person to review these.
Non-NHS Services
How to request private paperwork / signatures
Requests for the doctors or staff to sign paperwork, fill out forms and write reports for patients is often classed as private (non-NHS) work. This can include reports for insurance or solicitors, power of attorney forms, holiday cancellation forms, foster medical examinations, medical examinations for HGV or taxi drivers, private letters etc. We are only able complete private work for patients of The White House Surgery.
All such requests should be left with reception to pass on to the doctor. Do not book an appointment for the purpose of getting forms completed and signed. This will not be done during a consultation.
The partners reserve the right to charge a fee for all private reports, examinations and signatures that fall outside the NHS contract and any such work is only agreed entirely at the discretion of the partners. The fees depend on the amount of admin work and GP time involved in reading, researching, processing, completing reports, copying and tidying up. The reception team will be able to advise you of these fees on request.
Travel Vaccinations
Before Travelling:
Fill in the form as early as possible – at least 12 weeks before you travel
The nurse will review your health records and advise any vaccinations you may require. You will then be booked an appointment at the practice or if this is not a vaccination we can offer you will be sign-posted to an appropriate local service.
Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
Travel vaccines can also be sought through private local pharmacies or the Sheffield Health Travel clinic based at Royal Hallamshire Hospital, 01142712608 (Mon-Fri 9am-11am). Pre travel questionnaire.doc (live.com)
Travel vaccination forms can be collected from the reception desk.
